1. General Registration Renewal Reminders

- 2025 Registration Renewal closes January 7th
- If you uploaded your insurance memorandum prior to the Renewal period, you no longer need to upload it when you renew. Take care to upload your Memorandum of Insurance and not the receipt.
- If your CPR is current and uploaded at some time during the past year, you do not need to upload it when you renew.
- Please note your Renewal documents, i.e., CPR, must bear the same name that appears on your Profile page.
- If you need to complete a name change, please complete a profile update or include the request in your renewal. A Government Issued Marriage Certificate or Name Change document must accompany all name change requests.
- Please note if you are subscribed to the PAD (Pre-Authorized Debit) payment option for registration renewal, your fee will be processed from your account on December 12th, 2024. If you would like to enroll for PAD, or cancel your PAD, contact the CDHM office at cdhm@cdhm.info or 204-219-2678.
2. Criminal Record Check (CRC) with Vulnerable Sector Check
Most registrants submitted a CRC 5 years ago in 2019, and as per the Dental Hygienists Regulation 19 (1)(h), you are required to submit a valid Criminal Record Check, with a Vulnerable Sector Check for Renewal 2025.
CRCs have a 5-year renewal requirement.

- Winnipeg Police Criminal Record Checks can no longer be sent directly to the CDHM. Your Criminal Record Check will be sent to you by email or mail.
- If you are renewing with the Winnipeg Police, do not check that you have an account/agency even though you have requested the CRC from them previously.
NOTE: It is strongly suggested you complete your CRC well ahead of time because Winnipeg Police Record Checks can take up to two weeks to complete & RCMP CRCs can take up to 4 weeks. Due to the postal strike, there may be an interruption receiving your RCMP CRC. We are monitoring the situation and will grant extensions if necessary.
3. Indigenous Cultural Safety Module
Completion of the Indigenous Cultural Safety Module is a requirement for Renewal this year. The Module is accessed by signing in to your Profile page and clicking on Exams in the left margin. If ‘Pending’ is still showing, the Knowledge Check questions require completion before clicking the ‘Submit’ button. In acknowledgement of time spent completing the Indigenous Cultural Safety Module, a Reflection summary will not be required to claim the hour for your CC submission. On January 8th, 2025, after Registration Renewal is complete, you will be emailed a certificate which you can upload as proof of completion. The Module hour can be applied to your 2025 or 2026 CC submission.
In acknowledgement of time spent completing the Indigenous Cultural Safety Module, a Reflection summary will not be required to claim the hour for your CCP submission. On January 8th, 2025, after Registration Renewal is complete, you will be emailed a certificate which you can upload as proof of completion. The Module hour can be applied to your 2025 or 2026 CC submission.

5. Cancelling your Registration?
A CDHM Registration Cancellation Form is required to cancel your registration and keep your past registration in good standing. A Registration Cancellation form is available from the website. Once completed send the form by email or regular mail to the CDHM office.
