DECLARATIONS

If you have completed education in Local Anaesthetic, Restorative, or Orthodontic skills over the last 12 months, or have achieved 3000 career hours and wish to be included on the appropriate roster, please complete the appropriate declaration below. 

Declarations can be mailed or faxed to the CDHM office.  A new certificate and wallet card will be issued upon review of the declaration and accompanying certificates.
